A Physical Therapist (PT) in Home Health is a licensed healthcare professional who provides rehabilitation services to patients in their homes to improve mobility, strength, and overall function. They assess patients’ physical abilities, develop individualized treatment plans, and guide exercises to support recovery from injury, surgery, or chronic conditions. PTs in home health also evaluate home environments, recommend modifications, and educate patients and caregivers on safe movement and fall prevention. Working independently while coordinating with physicians and care teams, they play a key role in helping patients regain independence and avoid hospital readmissions.
