A Registered Nurse Case Manager (RN Case Manager) is a licensed nursing professional who coordinates and oversees patient care across the healthcare continuum. RN Case Managers assess patient needs, develop individualized care plans, and facilitate communication between patients, families, providers, and healthcare services. They manage utilization of resources, support discharge planning, and help ensure patients receive appropriate, cost-effective care. Working closely with physicians, social workers, and interdisciplinary teams, RN Case Managers advocate for patient needs while promoting continuity and quality of care. With strong clinical knowledge, organizational skills, and care coordination expertise, RN Case Managers play a critical role in improving outcomes and patient satisfaction.
