A Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) in a Long-Term Acute Care (LTAC) setting is a licensed nursing professional who provides hands-on care to patients with complex, chronic, or medically fragile conditions requiring extended hospitalization. LPNs/LVNs assist with medication administration, wound care, tube feedings, tracheostomy care, and monitoring of vital signs under the direction of registered nurses and physicians. They observe patient conditions closely, report changes promptly, and support daily care needs to promote comfort and stability. Working collaboratively with RNs, respiratory therapists, and interdisciplinary teams, LTAC LPNs/LVNs play a vital role in maintaining continuity of care and supporting recovery in a high-acuity, long-term setting.
